Become a Provider with CGHC
Common Ground Healthcare Cooperative offers an integrated care network to ensure that our members receive the highest quality, most cost-effective care. We work primarily with Aurora Health Care, Bellin Health, ThedaCare, Door County Medical Center, Watertown Provider and Hospital Organization, and Children’s Hospital and Health System providers.
We are not actively pursuing additions to our network at this time, but if you would like to submit a request for future consideration, please complete the network participation form, linked above.
