How to Read your Explanation of Benefits (EOB)

Your explanation of benefits (EOB) is a document that is mailed to you after receiving covered medical services. The EOB explains how the services you received are applied to your CGHC coverage. It explains the amount you owe to the provider, the amount your plan saved you, and explains your cost sharing. Please use this resource for more information:
