The Common Ground Healthcare Foundation mission:
Enhancing health and prosperity in the communities where Common Ground Healthcare Cooperative’s members live, work, learn, and play.
Common Ground Healthcare Cooperative formed the Foundation to expand its ability to partner with community organizations that are helping people gain a foothold when they meet an obstacle with their health or finances.
The Foundation will accept applications for funding from September 1st, 2024 to November 1st, 2024. Typical grant awards will be between $5,000 and $100,000. To be eligible, programs must directly or indirectly serve individuals in at least one of the following counties: Brown, Calumet, Dodge, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Marinette, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara, or Winnebago.
